• Req If ticked the question is required to be answered
• Long Description Additional details relative to the question that are displayed at time on entering Questionnaire answers

ANSWER OPTIONS:

• Description Description of the answer to be provided by the respondent
• Score Score given to the answer, used in calculating the value of the answer
• Order The order in which the answers appear
• Default Determines if the answer is the default answer
